Informed Consent, Liability Waiver, Medical Authorization, and Media Release

 

I, the undersigned parent or legal guardian, hereby give permission for my child to participate in all activities, practices, training sessions, competitions, conditioning, travel, and events associated with the Joelton Wrestling Club (“JWC”).

I understand and acknowledge that participation in wrestling and related athletic activities involves inherent risks, including but not limited to serious bodily injury, permanent disability, paralysis, illness, communicable diseases, concussion, emotional injury, property damage, or death. I voluntarily assume all known and unknown risks associated with my child’s participation in JWC activities, including risks arising from contact with other participants, equipment, facilities, transportation, and athletic activities.

In consideration for being allowed to participate, I hereby release, waive, discharge, and hold harmless the Joelton Wrestling Club, its board members, coaches, instructors, volunteers, representatives, sponsoring organizations, affiliated participants, facility owners, landlords, and event staff from any and all claims, liabilities, demands, actions, damages, costs, or expenses arising out of or related to any injury, illness, loss, damage, or accident sustained by my child during participation in JWC activities or events, including travel to and from such activities, except where prohibited by law.

I further acknowledge and agree that certain Joelton Wrestling Club activities, camps, practices, open mats, or events may be hosted, conducted, or held at facilities owned, operated, or maintained by third parties, including but not limited to First Baptist Church Joelton and the Metropolitan Government of Nashville and Davidson County, specifically Paradise Ridge Community Center. In consideration for my child being permitted to participate in activities at these facilities, I hereby release, waive, discharge, and agree not to hold liable First Baptist Church Joelton, Metropolitan Government of Nashville and Davidson County, Paradise Ridge Community Center, and their respective employees, agents, representatives, staff, volunteers, and affiliates from any and all claims, liabilities, damages, injuries, losses, or causes of action arising out of or related to my child’s participation in JWC activities conducted at those facilities, except where prohibited by law.

I understand and acknowledge that parents and guardians are solely responsible for arranging transportation for their child to and from JWC activities unless otherwise specifically arranged. In situations where transportation is voluntarily coordinated between parents, guardians, coaches, or other individuals, I acknowledge that such transportation is not provided or controlled by JWC. I agree not to hold JWC, its coaches, volunteers, board members, participating families, or drivers liable for any accidents, injuries, damages, or losses that may occur during such transportation, except where prohibited by law.

I certify that my child is physically capable of participating in wrestling activities and that I will disclose any relevant medical conditions, allergies, injuries, medications, or limitations to the club. I understand that suspected concussions, illnesses, or serious injuries must be reported immediately and that coaches or club representatives may restrict participation until appropriate medical clearance is provided.

I acknowledge the risk of exposure to communicable illnesses, including but not limited to COVID-19, influenza, MRSA, skin infections, and other contagious conditions commonly associated with athletic participation.

In the event of a medical emergency, I authorize JWC representatives, coaches, or volunteers to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am solely responsible for maintaining medical insurance coverage for my child and for any medical expenses incurred.

JWC reserves the right to suspend or remove any participant or parent whose behavior is deemed unsafe, disruptive, abusive, threatening, inappropriate, or detrimental to the club environment or the safety and well being of others.

I also grant permission for the Joelton Wrestling Club to photograph and/or record my child during club activities and events. I authorize JWC to use such photographs, video recordings, or likenesses for promotional, educational, advertising, website, social media, fundraising, and other club related purposes without compensation. I understand that if I do not wish for my child’s image or likeness to be used, I must submit a written request to the Joelton Wrestling Club Board, and JWC will make reasonable efforts to honor that request moving forward.

This agreement shall be governed by the laws of the State of Tennessee.

By signing below, I acknowledge that I have carefully read and fully understand this waiver and release. I understand that I am giving up certain legal rights on behalf of myself and my child, and I voluntarily agree to the terms stated herein.

Club Participation & Conduct Policy

This policy outlines the expectations for participation in the Joelton Wrestling Club (“JWC” or “Club”). By participating in club activities, practices, events, or competitions, all athletes and parents agree to comply with the standards outlined below.

1. Participation Expectations; AAU Membership

Wrestlers are strongly encouraged to attend practices regularly, arrive on time, and be properly prepared for training. Consistent participation is important for skill development, safety, and team progression. Families should communicate absences to coaching staff when necessary. Wrestlers must obtain their AAU Membership card ($30) directly from the AAU to participate in practices. This covers insurance and allows the wrestler to participate in AAU sanctioned tournaments.

2. Arrival, Pickup, and Supervision Responsibility

Parents or guardians are responsible for timely drop off and pickup of athletes. Wrestlers should try arrive 10 minutes early to ensure they are prepared for practice and to assist with mat setup. Parents are invited to remain during practice. JWC is not responsible for supervision before or after scheduled practice times. If a parent does not intend to remain at practice, or must leave the facility during practice, proper communication with coaches and the athlete is required.

3. Behavior Standards

All athletes are expected to demonstrate respect toward coaches, teammates, parents, and facilities at all times. Disruptive, unsafe, or disrespectful behavior will be addressed by coaching staff using appropriate corrective action, which may include removal from practice or further disciplinary measures if necessary. Parents and siblings are also expected to behave in a safe, undisruptive manner, and respect the facilities.

4. Coach Authority

Coaches are responsible for maintaining a safe and productive training environment. All coaching decisions regarding training, discipline, athlete pairing, and participation are final. Coaches have full discretion to make decisions based on safety, skill development, and the needs of the program.

5. Health, Safety, and Medical Responsibility

Athletes are expected to participate only when healthy and free from contagious illness or unsafe medical conditions. Parents are responsible for disclosing relevant medical concerns, injuries, allergies, or limitations. Participation is voluntary, and parents assume responsibility for ensuring their child is medically cleared for participation. Athletes may be restricted from participation if safety concerns are present.

6. Hygiene and Equipment Requirements

Athletes must arrive clean and properly groomed for all practices and events. Wrestlers should wear appropriate athleticwear, including gym shorts and t-shirts, or sweatpants and sweatshirts. Clothing should not have zippers, buttons, or other hard fasteners. No earrings or jewelry. Wrestling shoes are required on the mat. Headgear is required for all live wrestling. Notwithstanding the foregoing, wrestlers do not need headgear and shoes for their first practice; however, if the wrestler will continue to attend, they must have shoes and headgear. JWC singlets are required for competitions and sanctioned events, which may be purchased from the JWC-approved supplier. Mouthguards are required for athletes with braces. DO NOT WEAR WRESTLING SHOES OUTSIDE OF THE FACILITY.

7. Financial Responsibility

Although the Club strives to provide wrestling instruction at no cost, certain events may require fees. All fees and payments are non-refundable unless otherwise approved by club leadership due to exceptional circumstances. Financial assistance may be considered on a case by case basis.

8. Communication

The Club uses a private group chat as the primary method of communication for schedule changes, updates, and announcements. Parents are responsible for staying informed through this communication channel. Scheduling and other important information may also be shared on the Club’s Facebook page.

9. Tournament Travel and Responsibility

Tournament participation is optional but encouraged. Parents are responsible for all tournament registration fees, travel, lodging, transportation, supervision, and attendance at tournaments unless otherwise specifically arranged. Coaches will provide event details and guidance, but are not responsible for transportation or supervision outside of coaching duties at events. AAU Membership is required for tournament participation.

10. Facility Respect

All athletes and families are expected to respect training facilities, equipment, and host locations, including Paradise Ridge Community Center and First Baptist Church Joelton. Damage or misuse of facilities may result in removal from the program and potential financial responsibility.

11. Policy Agreement

Participation in any JWC activity constitutes agreement to this policy in addition to the Informed Consent, Liability Waiver, Medical Authorization, and Media Release. Club leadership reserves the right to interpret and enforce this policy in the best interest of athlete safety, development, and program integrity.
